Dear Parent/Guardian,

As the Lead Nurse, my first concern is the welfare of each student.  To be able to assist the student during an illness or an emergency, it is important for you to complete the following forms and return to the school nurse:

• HEALTH OFFICE EMERGENCY FORM - This form must be completed by the parent/guardian. Phone numbers will not be accepted from students. Parents are strongly encouraged to update this information as changes occur throughout the year. Phone numbers may also be updated by contacting the front office staff.

  • If a student becomes ill and is unable to return to class, we will attempt to reach the parent/guardian and then the emergency contacts listed on the Emergency Form. If there is an event of medical emergency, EMS services will be notified and utilized if necessary.

• HEALTH HISTORY FORM - Please complete and sign this form in order for the nurse to meet any health/medical needs your student may have. Please submit any additional care plans or health documentation that will help support the care that need to be provided.
